Abstract

Objective :
To report the results of hiatal hernia management at Niamey National Hospital.

Patients and method :
This was a retrospective series of cases collected over a 5-year period involving adult patients who had undergone surgery for hiatal hernia at the HNN.

Results :
The series included six patients. Five were male and one female, giving a sex ratio of 5:1. The mean age of the patients was 34.17 ± 5.77 years, with extremes of 27 and 42 years. Tea and coffee consumption was found in 83.34% (n=5). Epigastralgia was the most frequent reason for consultation in 66.7% of patients, followed by gastro-oesophageal reflux in 2 cases (33.33%). Oesogastroduodenal fibroscopy was performed in 83.34% of patients (n=5). Slipped hiatal hernia was the most common type of hiatal hernia found in 83.34% of patients (n=5), with 33.33% (n=2) having peptic oesophagitis (with 16.7% of patients at Savary Muller grades 1 and 2). Eighty-three point three percent (83.84%, n=5) of patients had undergone fundoplication according to Nissen and 16.7% of patients (n=1) had undergone fundoplication according to Toupet. The immediate post-operative course was straightforward in 83.34% of patients (n=5). One case of postoperative dysphagia was recorded. The average post-operative hospital stay was 4.5 days, with extremes of 5 and 7 days.

Conclusion :
Hiatal hernia remains rare in Niger. Surgical treatment is based essentially on fundoplication with satisfactory results.
