Abstract
Introduction :
ischaemic priapism is an uncommon andrological emergency, the main etiologies being sickle cell disease and abuse of aphrodisiac substances. Treatment aims to achieve rapid detumescence by intra-cavernosal administration of an alpha-stimulant drug, sometimes combined with cavernospongiosal shunting. The aim of this study is to present our experience in the management of ischaemic priapism.
Material and method :
This was a retrospective descriptive study from January 2015 to June 2023, including all patients admitted to the emergency department of the National Hospital of Zinder (HNZ) for priapism who had received treatment. The number of episodes per patient was not considered in this study.
Results :
A total of 22 patients were enrolled with a median age of 23.86 years (7-53 years). The mean time to consultation was 23.31 hours (extremes 3 hours and 82 hours) and more than 63% were admitted before the 10th hour. The main etiologys were sickle cell disease (59.10%, n=13), aphrodisiacs (31.82%, n=7) and idiopathic disorders (9.10%, n=2). All patients had undergone cavernous body lavage with intra cavernous injection of Etilefrine. Detumescence was complete in 82% (n=18) of cases and incomplete in 18% (n=4).
Conclusion :
Sickle cell disease and erectogenic drugs are the main causes of priapism in our study. Puncture and cleaning with saline of the corpora cavernosa combined with intracavernosal injection of alpha-stimulant drugs gave good results within 72 hours.
