Abstract

Introduction :
Respiratory distress is a life-threatening medical emergency. The aim of this study was to investigate respiratory distress in children aged 0-15 years admitted to the pediatric wards of the Amirou Boubacar Diallo National Hospital in Niamey.

Patients and methods :
This was a retrospective descriptive study covering one year from January to December 2020. The study took place in the pediatric ward of the Amirou Boubacar Diallo National Hospital in Niamey. We included children aged 0 to 15 years who consulted for respiratory distress and who had undergone a minimal assessment.

Results :
343 cases of respiratory distress were recorded out of 10,960 pediatric consultations, representing a frequency of 3.13%. Infants accounted for 63.85%. Males predominated, with a sex ratio of 1.41. The main symptoms were hyperthermia, cough and dyspnoea in 58.48%, 44.02% and 42.86% respectively. The most frequent signs of respiratory struggle were nasal flaring (63.56%), intercostal indrawing (56.27%) and thoracoabdominal rocking (45.48%). The main respiratory etiologies were bronchiolitis/asthma (30.32%) and pneumonia (21.87%). Emergency procedures were oxygenation (60.64%) and nebulisation (33.53%). Mono-antibiotic therapy was used in 71.85% of cases. The outcome was favourable in 85.13% of patients, with a mortality rate of 14.87%. The latter was associated with the mothers' lack of education (P= 0.017) and the presence of severe anaemia, bronchiolitis/asthma and severe malaria (P< 0.001).

Conclusion :
Respiratory distress is common, with a high mortality rate. The main etiologies were malaria, bronchiolitis/asthma and pneumonia.
