Abstract
Introduction :
Intracardiac thrombosis remains a prognostic turning point associated with a high risk of ischemic and embolic events and an increased risk of mortality, whatever the underlying heart disease or general illness. The aim of this study was to determine the diagnostic and therapeutic aspects of intracardiac thrombosis at the Hôpital National Amirou Boubacar Diallo (HNABD) in Niamey.
Patients and method :
This were a cross-sectional study with retrospective (January 1, 2016 to December 31, 2021) and prospective (January 1, 2022 to June 30, 2022) data collection, i.e. 78 months in the cardiology department of the HNABD in Niamey. Patients hospitalized with intracardiac thrombosis confirmed by imaging were included in the study.
Results :
Two hundred and fifty-two cases (252) of intracardiac thrombosis were recorded. Males accounted for 75% (n=174), i.e. a sex ratio of 2.23. The mean age of patients was 50.1± 19.8 years. The age range [55 to 64 years] was in the majority with 19.4% (n=49). Orthopnea and edema of the lower limbs were present in 81% (n=204) of cases, and cough in 72.6% (n=183). Dilated cardiomyopathy (DCM) was the underlying pathology in 53.1% of cases (n=134). The mean left ventricular systolic ejection fraction (LVEF) was 38.48± 15.13% (extremes: 15 and 77%). LV thrombi were present in 66.67% (n=54). Anti-vitamin K drugs (AVK) were prescribed for 61.9% (n=156) of our patients, and low-molecular-weight heparin (LMWH) for 33.3%. The outcome was marked by 31 deaths (12.30%), with the remainder of patients improving.
Conclusion :
Thrombotic pathologies remain frequent in Niger. The anticoagulant therapy is the mainstay of treatment. Their evolution is still marked by embolic complications, with a high mortality rate.
