Abstract

Introduction :
Congenitally corrected transposition of great arteries is a complex and rare congenital heart disease characterized by a double atrioventricular and ventriculoarterial discordance. The objective of this work was to describe the natural history of a case of congenitally corrected transposition of great arteries in adult in our context.

Observation :
This was a 41-year-old patient admitted for dyspnea and lower limb edema. The onset of symptoms dates back three weeks with dyspnea associated with edema of lower limbs and headaches. The patient is known to have poorly followed hypertension. The clinical examination found WHO grade III hypertension, systolic murmurs and galloping noise at the mitral focus. Pulmonary auscultation found crackling groans at both pulmonary bases. Echocardiography revealed a congenitally corrected transposition of the great arteries with Ebstein’s anomaly. Medical treatment helped improve the symptoms.

Conclusion :
Congenitally corrected transposition of the great arteries is a complex congenital heart disease with the prognosis depends on the associated anomalies. Our patient who had Ebstein’s associated anomaly progressed relatively quickly towards heart failure. Diagnostic delay and the technical deficiency didn’t allow performing complete treatment.
