Abstract
Introduction :
Ischemic priapism is a rare uro-andrological emergency in which iatrogenic etiologies predominate, with neuroleptics ranking first. The aim of this study was to describe a case of priapism secondary to the concomitant administration of haloperidol and chlorpromazine in a patient treated for paranoid schizophrenia who had been off treatment for one year.
Observation :
Mr K.A.L, aged 49, married, had been treated since 2006 for paranoid schizophrenia and had been off treatment for one year. He was admitted to the psychiatric emergency department for insomnia, psychomotor instability and incoherent speech. The diagnosis of a relapse of paranoid schizophrenia was retained. He presented with ischemic priapism secondary to the concomitant administration of haloperidol and chlorpromazine at the time of admission. He was admitted to hospital and underwent drainage puncture of the corpora cavernosa and amisulpride. The evolution was marked by psychic stabilization and the onset of persistent sexual impotence.
Conclusion :
Ischemic priapism is a uro-andrological emergency that can occur in patients on neuroleptics. Hence the need for careful monitoring and clear information on priapism in all patients on neuroleptics.
