Abstract
Introduction :
Chest trauma is severe when it leads to respiratory and/or hemodynamic failure that can be life-threatening. The aim was to describe the epidemiological, diagnostic and therapeutic aspects of severe thoracic trauma (TTG) at the Niamey General Reference Hospital (HGR).
Patients and methods :
This was a descriptive cross-sectional study conducted over a period of 2 years and 7 months. Patients treated for TTG at the thoracic and cardiovascular surgery department of the Hôpital Général de Référence were included. The variables studied were epidemiological, clinical, radiological, biological, therapeutic and evolutionary data. Data were processed using Sphinx software version 5.1.0.4.
Results :
A total of 41 patients were enrolled and managed for severe thoracic trauma. The M/F sex ratio was 5:1. The mean age of the patients was 35.36 years. Road traffic accidents were the most common cause of trauma in 65.9% (n=27). In 46.3% (n=19) of cases, the TTG was associated with a polytrauma. Head trauma was the most frequently reported associated injury in 34.2% (n=13). Chest X-rays were performed in 83% (n=34) of cases, followed by chest CT scans in 65.5% (n=27). At thoracic level, hemothorax was the most frequent lesion in 44.4% (n=18) of cases, followed by sternum fracture in 29.4% (n=10) and hemopneumothorax in 25.9% (n=8) of cases. Thoracic drainage was performed in 65.9% (n=29) of cases. Mortality was 12.2% (n=5).
Conclusion :
Thoracic trauma, whether isolated or associated with polytrauma, can be serious and a reason for hospitalization. They are often caused by road accidents and often affect young males. Awareness of road accidents needs to be stepped up.
