Abstract

Introduction :
Perinatal asphyxia (PA) remains a public health problem in our developing countries. The objective was to study the epidemiological, clinical and prognostic aspects of perinatal asphyxia at the Maternal and Child Health Center (MCHC) in Zinder, Niger.

Patients and methods :
This was a cross-sectional study using secondary and primary data over a 17-month period from January 1, 2022 to May 31, 2023. All newborns with perinatal asphyxia Zinder, Niger. admitted to the neonatology department of the MCHC in Zinder during the study period were included.

Results :
During the study period, 5,111 newborns were admitted, of whom 536 had perinatal asphyxia, or 10.48%. The average age of the mothers was 20± 4 years. Mothers were housewives in 89.74% of cases, uneducated in 80.78% of cases and their socio-economic level was low in 55.97% d. They were referred in 70% of cases and 74% came from rural areas. Pregnancies were poorly monitored in 73.31% of cases. Vaginal delivery was performed in 59.89% of cases. The main obstetric complications were dystocia with 42.66%, eclampsia/preeclampsia with 31.79% and retroplacental hematoma with 8.42%. The sex ratio of newborns was 1.6. The APGAR score was < 4 at the 5th minute of life in 15.30%. Resuscitation beyond 10 minutes was performed in 9.89% of newborns. Seizures were represented in 40.25% of cases. Anoxic-ischemic encephalopathy was severe in 22.99% of cases. The average length of stay was 5.83 days. Maternal pathologies, poor pregnancy monitoring and anoxic-ischemic encephalopathy were associated with the occurrence of perinatal asphyxia (p < 0.001). The perinatal mortality rate was 10.45% of cases. There is an association between very low APGAR score (< 4) and the occurrence of deaths (p˂0.001).

Conclusion :
Perinatal asphyxia remains a serious pathology with a fairly high mortality at the CSME of Zinder. The contributing factors identified were maternal (toxemia), adnexal (HRP) and obstetric (dystocia). Maternal pathologies and APGAR < 4 at the 5th minute were the main causes related to deaths.
