Abstract
Introduction :
Gastrointestinal haemorrhage, a frequent reason for consultation, is a medical surgical emergency responsible for a fairly high morbidity and mortality. Digestive endoscopy has made it possible to better inform ourselves about the etiology of these diseases and thus to improve their management. The objective of our study is to provide an overview of gastrointestinal bleeding at the National Hospital of Niamey (Niger) in the era of diagnostic and therape utic digestive endoscopy.
Methodology :
this was an observational, prospective descriptive study from July 1, 2023 to December 31, 2023, i.e. 6 months. All patients hospitalized in the Hepatogastroenterology Department of the National Hospital of Niamey for gastrointestinal bleeding were included.
Results :
303 patients were admitted to the department, including 57 cases of gastrointestinal bleeding, i.e. a frequency of 18.81%. The average age was 32.3 years. The age group of [30 years - 39 years] was the most represented. Men were the most represented in 66,67% (sex ratio 2). Gastrointestinal bleeding was high and very abundant in 70.18% (n=40) and 31.58% (n=18) of our patients, respectively. Hematemesis was the most common mode of exteriorization with 57.89% of cases (n = 33). The time to hospital admission was less than 6 hours in 31.58% of patients (n=18). The hemodynamic status was unstable in 66.66% of patients (n=38). The etiology of upper gastrointestinal bleeding was dominated by rupture of oesophageal varices (55%) and that of lower gastrointestinal bleeding by haemorrhoidal disease (40%). Epigastralgia was the most associated clinical sign. 66.66% of patients were resuscitated. No patient benefited from endoscopic surgery. The course was favorable in 94.74% of cases (n = 54).
Conclusion :
Gastrointestinal bleeding is quite frequent, dominated by high hemorrhage with rupture of the oesophageal varices as the main etiology. The treatment consisted of resuscitation which made it possible to rectify the prognosis
