Abstract

Introduction :
Takayasu disease is a non-specific inflammatory arteritis of large- and medium caliber arteries. The great clinical polymorphism of this disease means that diagnosis is often made in its “occlusive” phase. Our aim was to describe a case of Takayasu disease revealed by an aortic dissection diagnosed at the Abidjan Heart Institute.

Observation :
Miss TA, 21 years old, female, with no notable pathological history, consulted for transfixing chest pain accentuated in the medio-thoracic region. Clinical examination revealed an apyretic patient, a soft abdomen and epigastric pain on palpation. Cardiovascular examination revealed blood pressure of 217/108 mmHg at BG, 174/95 at BD, and asymmetric peripheral pulses in the upper limbs, faintly felt in the lower limbs. Elsewhere, the clinical examination was unremarkable.

Conclusion :
Aortic dissection complicating Takayasu's disease is a rare association in sub Saharan Africa. Angioscan is an excellent diagnostic tool in view of its clinical polymorphism. Management was based on medical treatment and revascularization with stenting.
