Abstract

Introduction :
Heart failure is a serious condition that is frequently caused by a number of etiologies. Compliance with treatment is a major challenge. The aim of this study was to describe compliance with treatment among chronic heart failure patients followed up on an outpatient basis at the cardiology unit of the Hospital National de Niamey (HNN).

Methodology :
Prospective descriptive study of 130 patients followed as outpatients at the cardiology unit of the HNN. Data collection took place from 01/06/2021 to 04/01/2021. Microsoft Office 2019, Word, Excel and Sphinx plus were used for data entry and processing.

Results :
The incidence of heart failure was 10.80%. The mean age was 54.72±16.78 years and the age group 60 and overrepresented 47.46%. The sex ratio was 0.88, with females predominating at 53.10%. On clinical examination, dyspnea, cough and chest pain were the functional signs found most frequently in 70%, 28.46% and 18.46% respectively. General condition was preserved in 96.15% of cases. Tachycardia was found in 10%, SVT in 11.54% and IMO in 8.46%. LVH (68.46%), ESV (12.31%) and AF (5.38%) were the most common electrical findings. On ultrasound, LV dilatation (86.92%), LVEF ˂ 35% to 31.32% and hypertensive CMD (24.61%) were the main findings. Patients were treated with ACE inhibitors (88.46%), beta blockers (82.31%) and diuretics (79.23%). The average duration of follow-up was between 3 and 6 months in 50% of patients. We found a rate of good compliance in 54.62% and non adherence in 45.38%. The causes of non-adherence were financial insufficiency (28.81%), ignorance (27.12%) and forgetfulness (18.64%). There was a statistically significant relationship between convenience, flexibility, desire to continue treatment, state of health and drug intake, with P-values of 0.007, 0.023, 0.001 and 0.018 respectively.

Conclusion :
Heart failure is becoming increasingly common. Hypertensive DC was the main cause. Therapeutic compliance is unsatisfactory, mainly for economic reasons.
