Abstract

Introduction :
Niger is a tuberculosis-endemic country. Extrapulmonary localizations may be encountered. The aim of this study was to report a case of multifocal tuberculosis (orbital and pleural) in a 19-year-old soldier who had initially been vaccinated against tuberculosis.

Observation :
Mr. MO, 19 years old, with no particular pathological history, was presented with a painful red eye in a febrile setting. Bilateral palpebral edema, bilateral exophthalmos with red, painful and purulent eyes with impaired vision, resistant to the usual antibiotics. Pleuropulmonary examination revealed a pleural effusion syndrome with fluid on the right and gas on the left. Cyto-bacteriological examination of the pleural fluid revealed a predominantly lymphocytic exudate. Conjunctival secretions were swabbed negative for bacilli. Genexpert examination of pleural fluid revealed Rifampicin-sensitive Mycobacterium tuberculosis (MTB). The patient was started on a fixed-combination antituberculosis regimen (RHZE and RH). After 14 months, the exophthalmos had disappeared.

Conclusion :
The orbital localization of multifocal tuberculosis presents a great clinical polymorphism, making it necessary to evoke it before any orbital cellulitis resistant to nonspecific antibiotics.
