Abstract

Introduction :
Caustic ingestion is a medical and surgical emergency. It often leads to complications, the most common of which is stenosis. Management is multidisciplinary.

Objective :
To describe the management of caustic ingestion in Bangui. Patients and Methods: We conducted a 7-year cross-sectional analytical study, from January 1, 2017, to December 31, 2023, including patients under 18 years of age who had ingested caustic products and who underwent upper gastrointestinal endoscopy.

Results :
We collected 67 (47 men (70%) and 20 women (30). The average age was 34 years with extremes of 18 and 78 years. The ingestion of caustic was voluntary in 55 cases (82%) and accidental in 12 cases (18%). The causes of voluntary ingestion were family conflict in 85% and the loss of a valuable object in 15%. The main products ingested were sulfuric acid (37%) and caustic soda (34%). The main clinical signs were abdominal pain (77%), vomiting (61.2%) and odynophagia (52%). Digestive endoscopy had observed stage IIIb esophageal lesions in (27.1%). All patients were put on a strict diet Endoscopic control showed regression of caustic lesions in 15 cases and caustic stenosis in 15. The 15 patients with gastrointestinal tract stenosis underwent feeding gastrostomy (20%) or feeding gastrostomy (40%) and jejunostomy. The outcome was marked by improvement in general condition in 27% and death in 73%.

Conclusion :
Caustic ingestion is common, most often voluntary in young adults. It often progresses to stenosis, the management of which remains difficult in our context.
