Abstract

Introduction :
A systemic disorder, sickle cell disease presents multiple clinical manifestations including splenomegaly. The objective of this work was to study the characteristics of sickle cell patients presenting with splenomegaly at the Sourô Sanou University Hospital (CHUSS) in Bobo-Dioulasso.

Materials and methods :
This was a descriptive and analytical cross-sectional study with prospective data collection, which took place over a 7-month period from April 2023 to January 2024. All sickle cell patients in the stationary phase, from the Internal Medicine and Pediatrics departments, were included. Resultats: A total of 118 sickle cell patients were collected, of which 81 (68.64%) were from pediatrics and 37 (31.36%) were from internal medicine. Among the 118 patients, 14 (11.9%) had splenomegaly. The mean age of the patients was 13.72 ± 13.20 years with extremes of 0 and 74 years. The predominance of patients with splenomegaly was male. 85.71% of patients had a history of vaso-occlusive crises (VOCs), with an average of 2.5 ± 1.7 crises per year. The clinical signs accompanying splenomegaly were jaundice and hepatomegaly in 7.1% of cases each. Splenomegaly was diagnosed clinically by palpation in 9/14 patients, or 64.3%, of whom 33.3% were type 1, 55.6% type 2, and 11.1% Hackett type 3. Factors associated with splenomegaly were age, sex, and phenotype.

Conclusion :
The frequency of splenomegaly in sickle cell disease is high in our region. Moderate spleen volume is most often associated with male sex, age, and the SC phenotype.
