Abstract
Introduction :
Sickle cell disease is the most common haemoglobinopathy. Its progression is marked by complications, particularly cardiovascular damage, which is a major prognostic factor. The aim was to study the electrical and echocardiographic aspects in adult sickle cell patients under the age of 15 in Niger.
Methodology :
This was a prospective, descriptive, cross-sectional study conducted at the National Sickle Cell Disease Reference Centre (CNDR) and the cardiology department of the National Hospital of Niamey over a period of 1 year and 5 months. All adult sickle cell patients in the intercritical period were included.
Results :
61 patients were included, with a sex ratio of 0.92. The average age was 7.6 years, ranging from 1 to 14 years. SS and SC phenotypes accounted for 87% and 13% of cases, respectively. The average baseline haemoglobin level was 9.63 g/dl. Electrically, the abnormalities found were: sinus tachycardia, primary and secondary repolarization disorders, incomplete right bundle branch block, left atrial hypertrophy (LAH) and right and left ventricular hypertrophy (RVH and LVH) in 52%, 19.67%, 1.64%, 8.20%, 1.64%, 4.92% and 6.56% of cases, respectively. The echocardiographic abnormalities found were dominated by tricuspid regurgitation in 8.2% of cases. Other valvular defects, namely mitral, aortic and pulmonary insufficiency, were found in 2%, 2% and 1.6% of cases, respectively. Left atrial and left ventricular dilatation accounted for 7% and 10% of cases, respectively, and right ventricular dilatation for 5%. One case of atrial septal defect and one case of ventricular septal defect were observed.
Conclusion :
Major sickle cell disease is a severe condition whose prognosis is worsened by cardiac involvement, hence the need for early screening with appropriate follow-up.
