Abstract

Introduction :
Malaria remains a major parasitic disease in Niger, where it poses a significant public health challenge. Its diagnosis relies on microscopy (thick blood smear and blood film), whose reliability is crucial for effective patient care. The objective of the study was to perform an external evaluation and concordance of microscopic readings for the biological diagnosis of malaria. Materials and Methods: This cross-sectional, descriptive study with prospective data collection was conducted over a six-month period, from February 5 to August 4, 2024, in eight biomedical laboratories (three public and five private) in the city of Maradi. The study aimed to evaluate the quality of microscopic diagnosis of malaria through the analysis of 385 slides read by technicians and 385 control slides.

Results :
The results revealed that TBS is used systematically in 100% of laboratories, while the TS is rarely requested. Only 75% of laboratories applied a standardized operating procedure (SOP). The overall concordance between different microscopic readings ranged from 64.5% to 96.5%, indicating a need to strengthen skills and standardize practices.

Conclusion :
The study recommends written procedures and a national EQA program to improve the quality of malaria biological diagnosis, harmonize practices, and strengthen confidence in laboratory services.
