Abstract

Introduction :
Cystic lesions of the pancreas represent a heterogeneous group of pathological entities with a broad lesion spectrum. The objective of this study was to describe the diagnostic and therapeutic aspects of pancreatic cystic lesions.

Materials and methods :
This is a single-center retrospective descriptive study of 9 cases of pancreatic cystic lesions operated on at the Hassan II University Hospital of Fez over a two-year period (from January 1, 2023, to December 31, 2024). Clinical, imaging (CT, MRI, endoscopic ultrasound), and histopathological data were analyzed using the HOSIX database.

Results :
The cohort was exclusively female (n=9, 100%), with a mean age of 50±18.2 years (median: 52 years, interquartile range [IQR]: 35–65 years; range: 17–81 years). Epigastric pain was present in all patients (n=9, 100%). Lesions were predominantly located in the pancreatic tail (8/9, 88.9%). All patients underwent partial pancreatectomy. Histologically, benign mucinous cystadenomas (MCN with low-grade dysplasia) accounted for 4/9 cases (44.4%), and overall mucinous lesions (including 1 case of borderline tumor with high-grade dysplasia) represented 5/9 cases (55.6%). Two cases of solid pseudopapillary neoplasms (2/9, 22.2%) were confirmed by immunohistochemistry (CD10+, PR+). Anatomic-radiologic correlation was observed in 5/9 cases (55.6%). The immediate postoperative recovery was favorable for all patients.

Conclusion :
The management of pancreatic cystic lesions requires an integrated multidisciplinary approach. In our retrospective surgical cohort, surgical resection of benign and borderline forms yielded excellent immediate postoperative outcomes.
