Abstract
Introduction :
Pleomorphic adenoma of the lacrimal gland is a rare orbital tumor whose definitive diagnosis is based on histopathological examination. The objective of this study was to report three cases managed in the Department of Maxillofacial Surgery at Yalgado Ouédraogo University Hospital in Burkina Faso.
Cases :
The patients were three women aged 27, 32, and 75 years, who presented with a superolateral orbital mass that had progressed gradually over 6 to 18 months. Clinical examination found unilateral exophthalmos, left-sided in two cases and right-sided in one case, associated with a firm, painless mass fixed to the deep plane. Diplopia was present in two patients. Orbital computed tomography showed an expanding tissue process arising from the lacrimal gland and displacing the eyeball, with one case in an intraconal location. All patients underwent complete en bloc surgical excision without prior biopsy through an upper eyelid approach. Histopathological examination of the surgical specimens confirmed the diagnosis of pleomorphic adenoma of the lacrimal gland. The postoperative course was uneventful. After a 6-month follow-up, no local recurrence was observed, and the clinical outcome remained favorable in all patients.
Conclusion :
Pleomorphic adenoma of the lacrimal gland requires complete en bloc excision without prior biopsy in order to reduce the risk of recurrence and malignant transformation. Long-term follow-up remains necessary despite an initially favorable outcome.
