Abstract

Introduction :
Wolff Parkinson White syndrome (WPW) is a congenital anomaly of the conduction pathways due to the existence of an accessory atrioventricular pathway bypassing the normal conduction pathway: the bundle of Kent. Atrial fibrillation is sometimes the initial arrhythmia in WPW syndrome, its occurrence is a risk factor for sudden death. We report the case of paroxysmal atrial fibrillation revealing a Wolf-Parkinson-White syndrome received at the referral hospital of Maradi Clinical case: This is a 63-year-old patient seen in consultation for the palpitations, dyspnea NYHA class 4 associated with orthopnea and hemodynamic instability. The per-critical ECG had revealed rapid atrial fibrillation 180 beats per minute with wide complex QRS, the emergency management of which was external electrical cardioversion at 200 joules, which allowed a return to a sinus rhythm on the accessory pathway. The evolution was favorable, the patient was placed under medical treatment and treatment by ablation of the accessory pathway was proposed.

Conclusion :
Paroxysmal atrial fibrillation is often the first manifestation of WPW syndrome in people over 60, demonstrating the Wolf-Parkinson-White polymorphism.
