Abstract
Introduction :
Hepatocellular carcinoma (HCC) is a common complication of chronic liver disease. Its weight is enormous in the context of the high endemicity of the hepatitis B virus. The objective of this work was to describe the epidemiological and prognostic aspects of hepatocellular carcinoma. Materials and Methods: This was a cross-sectional study with retrospective data collection from January 01, 2021 to December 31, 2023 on the records of patients followed on an outpatient basis and/or hospitalized in the Hepato-Gastroenterology Department (HGE) of the Zinder National Hospital (HNZ) for HCC.
Results :
there were 514 cases including 275 cases of chronic liver disease, among which there were 113 cases of hepatocellular carcinoma, i.e. 21.98% of reasons for hospitalization. The mean age was 42.02±16.26 years with a predominance of men (sex ratio at 1.33). The reasons for hospitalization were deterioration in general condition (71.75%), abdominal pain (32.82%), abdominal mass (12.98%), hepatic encephalopathy (12.98%) and gastrointestinal bleeding (12.98%). Patients were classified BCLC C/D in 100% (n=131). The alfa-fetoprotein level was greater than 500 ng/ml in 86.26% (n=113). Viral hepatitis B and its markers accounted for 87.78% (n=115) of the etiologies. The evolution of patients was marked by the occurrence of complications in 68.7% (n=90) and death in 85.5% (n=112) of patients. Complications were hepatic encephalopathy, gastrointestinal bleeding and infection of the ascites fluid in 52.67% (n=69), 14.50% (n=19) and 1.53% (n=2), respectively.
Conclusion :
HCC is a common cause of hospitalization. Diagnosis is late and the prognosis is serious. The main etiology is hepatitis B virus infection. Adequate management of HCC includes vaccination against this virus and monitoring of chronic liver disease.
