Abstract

Introduction :
The innominate artery aneurysm (IAA) is a rare entity. The objective of this study was to report a case of a late stage diagnosis of IAA in a context of limited resources and conduct a literature review of this unusual clinical condition.

Observation :
This was a 40-year-old woman who consulted for pulsatile swelling of the right cervicothoracic area at the surgical department at the National Hospital in Zinder, Niger. The medical history revealed a tracheal and esophageal compression, a cervical pain and right arm hypoesthesia. Cervico-thoracic CT scan and Doppler ultrasound showed a large oval opacity, right paratracheal, characteristic of a partially thrombosed IAA. The evolution was marked by the death of the patient one week after her admission to the hospital, following a rupture of the aneurysm.

Conclusion :
The innominate aneurysm is a serious and fatal condition when the diagnosis and surgical management are not early. The challenge remains important in our context where diagnosis is late (pre-rupture or thrombo-embolic complications) and the technical platform for management is lacking.
