Abstract
Introduction :
Acute lymphoblastic leukemias (ALL) are malignant hemopathies. It is the most common cancer in children under 15 years old. The objective was to evaluate the OP protocol (Oncovin and Prednisone) in the palliative management of ALL. Patients and Methods: this is a retrospective, descriptive study with an evaluative aim, going from January 2014 to August 2018. This study concerned all the patients followed in the Clinical Hematology Department of the Yopougon University Hospital (Abidjan) for ALL under palliative treatment.
Results :
The mean age of the patients was 33.9 ± 20.25 years (range: 2-76 years). There was a male predominance with a sex ratio of 1.29. The mean time to consultation was 73.2 ± 49.05 days (range: 7-180 days). Hyperleukocytosis was the main reason for consultation (84.6%) with deterioration in general condition (WHO performance status3 in 46.2% cases). There was a predominance of anemic syndrome (94.8%) and severe thrombocytopenia between 0 and 25G / l was observed in 41.0% of cases. The myelogram showed a predominance of cytological type LAL2 in 51.3%. The majority of our patients were predicted to be High risk in 82.1%. The mean survival of patients on palliative OP treatment was 94 days, significant compared to the mean survival without palliative OP treatment which was 20 days.
Conclusion :
The palliative approach using the vincristine-prednisone (OP) protocol improves average survival. This palliative care of patients in advanced or terminal phase of leukemia, which still remains to be studied, can be used in our settings with limited resources.
