Abstract

Introduction :
Anglo-Saxon “handbar hernia” or “handlebar hernia” is a traumatic parietal hernia resulting from direct blunt trauma by a handlebar or similar object on the abdominal wall. The objective of this study was to report the management of two (2) cases of strangulated “handlebar” hernias.

Observations :
There were two (2) children, one 5 years old and the other 7 years old admitted to the surgical emergency department of the Regional Hospital Center (CHR) of Maradi, for diffuse abdominal pain, febrile with arrest materials and gases evolving for 72 hours. The clinical history found a notion of trauma between 4 and 7 days. The inspection showed an irreducible painful swelling next to the point of impact and the palpation noted a sensitive abdomen for the 2 patients. The indication for laparotomy was made for strangulated traumatic hernias. The exploration made it possible to discover for the 2 cases, an ileal perforation and a parietal breach. A suture with absorbable 1/0 suture of the breach was made after freshening of the edges. An ileostomy was made for one case and an ileal suture for the other. The postoperative course was simple and the restoration of digestive continuity was carried ou

Conclusion :
Anterior abdominal HPT should be suspected especially in front of a deceleration mechanism. It can be strangled and sometimes associated with intestinal perforation creating a picture of peritonitis.
